Membership Form

I acknowledge a full understanding of the inherent dangers and risks associated with the use of the facilities and equipment at Evolve Fitness and Training and/or any fitness/wellness activity occurring therein. I also acknowledge that it is my responsibility to follow any instructions for any activity or use of equipment. I further understand that, notwithstanding precautions taken by Evolve Fitness and Training, my use of, presence in, or participation in activities conducted by Evolve Fitness and Training involves a risk of injury and/or death. I am voluntarily participating in these activities and voluntarily using equipment, and I accept any and all risks associated with my participation in activities conducted at Evolve Fitness and Training.

I acknowledge it is recommended that I seek approval from my physician before implementing an exercise regimen, as there may be significant health risks associated with exercising. 

In consideration for being permitted to use the facilities and equipment at Evolve Fitness and Training and because I assume all risks involved, I hereby agree that I am responsible for any resulting personal injury or illness, damage to, or loss of my property arising out of my attendance at Evolve Fitness and Training, or participation in any fitness/wellness activity occurring therein.

I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Evolve Fitness and Training, its owners, and/or employees (“Releases”) from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of my use or occupancy of Evolve Fitness and Training or any fitness/wellness activity occurring therein, including any injuries arising from negligence of the Releases or otherwise, to the fullest extent permitted by law.

I further state that I am at least eighteen (18) years of age and fully competent to sign this document, or will have a legal guardian sign attesting to the same. I agree to pay all applicable fees, and I execute this waiver and release for full, adequate, and complete consideration fully intending to be bound by the same.

1. Use facilities and equipment at your own risk.
2. Use equipment and machines properly and follow directions carefully. Please ask if you do not know how to use something.
3. Any equipment damaged by blatant abuse will result in a charge to that member to repair/replace the equipment. 
4. Report any damaged equipment immediately to the front desk. Do not use damaged equipment. 
5. Return all equipment to its appropriate place after use including plates, dumbbells, bands, barbells, mats, etc. Failure to return equipment will result in a warning the first offense and then a $20 fine each offense afterwards.
6. Do not drop weights, including dumbbells. Please guide all weights to the floor.
7. Clean machines/equipmemt after each use and properly dispose of trash. 
8. Evolve Fitness and Training is not responsible for lost or stolen personal items. Anyone caught stealing will be asked to leave and the appropriate legal actions will be pursued. 
9. Be mindful and respectful towards others while training. No horseplay.
10. All guests must have a signed waiver and pay the day pass fee (unless using the "First Day Free" trial). Guests are only allowed during staff hours or with approval from the owners. Bringing a guest inappropriately will result first in a warning and then a revoked membership.

Any individual found breaking these policies may be asked to leave the gym and/or have their membership revoked.
